Terms and Conditions for Football Application Form
- Introduction
By submitting this application form, you (the applicant or parent/guardian if under 18) agree to abide by the terms and conditions set forth by Afeez Football League.
- Eligibility
- Applicants must meet the age and skill requirements specified by the League Management.
- Submission of the form does not guarantee acceptance into the League.
- The League Management reserves the right to accept or reject applications based on criteria such as fitness level, skill assessment, and available slots.
- Other clubs might show interest in the same Town /and Cities you are applying for, to qualify, the pre-qualify game will be play among the rest and the highest point scorer will represent the Town /Cities you choose.
- Code of Conduct
- Players must adhere to the League Management’s rules, including respect for coaches, teammates, referees, and opponents.
- Any form of misconduct, including violent behaviour, discrimination, or cheating, may result in suspension or expulsion.
- Players are required to attend training and matches as scheduled.
- Fees and Payments
- Registration and membership fees (if applicable) must be paid as per the League’s payment policy.
- Fees are non-refundable except in special circumstances as determined by the League Management.
- Late payments may result in suspension from training and matches.
- Medical Fitness and Liability
- The applicant confirms that they are medically fit to participate in football activities.
- A medical certificate may be required before participation.
- The League Management is not responsible for any injuries, illnesses, or accidents that occur during training, matches, or Club-related activities.
- Players/Guardians must have appropriate insurance coverage for medical expenses. But not necessarily if which chose not to have one.
- Use of Image and Likeness
- The League Operator may use photos, videos, or other media featuring players for promotional purposes, including on social media, websites, and print materials.
- If you do not consent to this, please notify the League Management in writing.
- Data Protection
- Personal data collected in this form will be used solely for administrative and Club-related purposes.
- The League Management will not share personal details with third parties without consent, except as required by law.
- Termination of Membership
- The League Management reserves the right to terminate a player’s membership due to disciplinary reasons, non-payment of fees, or failure to comply with League policies.
- Players may withdraw voluntarily, but any outstanding fees must be settled before departure.
- Parental/Guardian Consent (For Minors)
- Parents or legal guardians must consent to the participation of applicants under 18.
- They acknowledge responsibility for the player’s conduct and adherence to Club policies.
- Amendments
- The League Management reserves the right to update or modify these terms at any time.
- Continued participation implies acceptance of any updated terms.

Important Notice – Afeez Football League
- Eligibility: Applicants must be between the ages of 16 and 21. Applications from individuals outside this age range will be rejected.
- Application Deadline: All applications must be submitted by October 27, 2025. Late submissions will not be considered.
- Post-Deadline Meeting: After the application deadline, the Afeez Football League Management will send an official letter or email to all club managers or team leaders inviting them to a get-together meeting to discuss a smooth transition for the league.
- League Kick-off: The league will officially start in 2026. The exact date and additional programs will be discussed with club managers or team leaders during the meeting.
